 ABOUT US | MEDICAL FORMS No visit to a physician would be complete without paperwork. We recognize that your time is valuable and that you may have difficulty filling out paperwork because of problems with your hands. In order to facilitate your registration, and perhaps allow a friend or family member to assist you, we have links to PDF forms below. If you would like to print out the relevant form or forms and complete them before your appointment, this will save you time when you arrive at our office. All patients will need a privacy notice and three other forms (privacy notice receipt, financial policy acknowledgement, medical history form). Please see the instructions below regarding which additional forms you may need to fill out. |
